Hayley Graves

During my time as a case manager, I have successfully case managed very complex and catastrophic injury cases with compassion, empathy and a timely approach to need. I have been able to navigate around very complex cases and successfully work through any barriers to rehabilitation, maintaining a client centred approach. Prior to my role as a case manager, I was an experienced social worker holding a high volume and complex case load in senior positions within the public sector. I am experienced in working with children following acquired brain injury and traumatic brain injury, sensory impairment, visual and hearing impairment, behavioural difficulties, Autism and ADHD, attachment disorder, Cerebral Palsy and birth injuries and have high levels of competence when assessing and implementing an appropriate support network and rehabilitation plans for children and their families.

My approach is collaborative and goal focused ensuring creative ways to meet clients' needs and progress rehabilitation to a ensure meaningful, well-co-ordinated and a progression monitored approach, striving to achieve the best possible outcomes for children and their families. My passion and commitment to this role led to excellent outcomes for the clients I work with. Previously working in a senior social work position, I am well versed in Section 17 needs assessments and section 47 Safeguarding assessments and accomplished in chairing multi-agency meetings and managing conflict amongst both families and professional networks. I use the latest best practice guidance and tools within my work and remain current through CPD and networking.

I understand the importance of sourcing a strong multi-disciplinary team to provide a holistic and structured approach to therapy and rehabilitation, drawing on professional expertise to provide client centered bespoke, cost-effective service that meet all aspects of the clients needs. I have a good understanding of the Education Health and Care Plan process and can support children with additional needs and their families through this process and challenge areas where needs are not being met.

I provide comprehensive Immediate Needs Assessments and update reports, inclusive of costings to support legal teams throughout litigation periods and post settlement. As part of case management services, I provide emotional support to the client and family throughout this period.

I am approachable and take pride in building trusting professional relationships with clients and professionals to create a strong, well nurtured team around the child. Providing quality case management and being part of a team who empower independence, create and support realistic, achievable goals and seeing a client’s quality of life improve is my passion.

Anthony Graves

I work alongside Hayley to provide essential administrative and financial operations. I have 17 years expereince in the financial sector and a strong eye for detail when managing day to day administration and operations.

I have qualifications in music and media, I.T and graphic design bringing creativity, knoweldge and precision to my role, which includes managing documentation, coordinating appointments, managing invoicing and ensuring smooth day to day operations, allowing Hayley to focus on delivering quality case management to our clients.

My excellent organisational skills ensure that HG  Case Mangement operates flawlessly by playing a key role in maintaiing efficent client centered case managemnnet services. Alongside this role I have expereince with paediatric brain injury, visual and senosry impairment and how this impacts on daily life.
